Signing up for Coleman Funeral Home's Local Serviceperson Benefit Program is easy. Download this form, print it off, complete it, and send it to us by fax or mail.

Who is eligible?

Members of the following departments (including administrative personnel) who are recognized by their department head as current, active employees up to age 65:
  • Police and fire departments of Oxford, Olive Branch, Southaven, Horn Lake and Hernando
  • Sheriff ’s departments and E-911 offices of Lafayette and DeSoto counties
  • Lafayette Co. Volunteer Fire Dept.
  • University Police Department
  • DeSoto County fire departments: ACI, Bridgetown, Eudora, Fairhaven, Lewisburg, Love, Nesbit and Walls
  • Emergency management offices of Lafayette and DeSoto counties
  • Emergency medical personnel in Lafayette and DeSoto counties

What would this program cover upon my death?

Essentially, complete care for your visitation, funeral and burial services.

This program entitles you receive at no charge all the services provided under Coleman Funeral Home’s Traditional Value Funeral Service Package. Benefits include the services of our professional staff; the use of our facility for both visitation and service; several choices among caskets; a graveliner vault; and transportation for the family. Should you prefer cremation, you or your family may subsitite the Complete Cremation Package at no charge.

Does this program only cover funeral expenses if I am killed in the line of duty?

No. As long as you are actively employed and enrolled in this program, you are covered regardless of the cause of death or whether it occurs when you are on duty or off duty.

Can I keep these benefits when I retire or resign from my department?

This program is only for active members. Please contact Coleman Funeral Home when you retire or resign so that we can ensure you have funeral coverage to protect your family.

What if I or my family end up wanting to use another funeral home?

That’s completely fine. The use of these benefits is completely optional, as this program is our gift.
